feat(agent): project wire tool names per model family

Summary

• Projects wire tool names per model family (default vs gpt profiles) so model families receive their native tool naming conventions while engine IDs remain unchanged.
• Projects Codex-compatible update_plan schema when aliasing manage_tasks for the gpt profile.
• Accepts all advertised names across profiles on incoming tool calls so dispatch, grants, and history replay remain profile-independent.

Verification

• bun run typecheck, bun run build, and bun test ./src pass cleanly.
• src/agent/tool-aliases.test.ts asserts per-family tool profiles, dispatch round-trips, and plan schema translation.
