Fix broken star history chart - #425

The star history chart is currently broken due to GitHub stargazer API restrictions, so the README and all localized READMEs now point to a working alternative. Also switch the chart image endpoints from /chart to /svg and the wrapping links to hash-based URLs.

The star history chart in the READMEs is currently broken because of GitHub stargazer API restrictions. This change points the chart to a working alternative so the chart renders again in the main README and all localized README versions.
