Persist Cardholder's Name in the db && optional debugging!

To Test Cardholder Name

1. execute a transaction
2. check the instrument table in the database
3. confirm the cardholder's name you provided is persisted under the name column

To Test Optional Debugging

1. Set DEBUG_MODE in your .env file to "true"
2. docker up
3. Confirm the debug environment is functioning
4. Set DEBUG_MODE in your .env file to "false"
5. docker up
6. Confirm debug environment is not available or running

Note this PR also includes code from #140 so it may be wise to also confirm those test procedures as well.
