bug: running a skill slash command displays full SKILL.md content as the user message

When a skill is invoked via a slash command (e.g. /my-skill), the entire contents of the skill's SKILL.md file appear verbatim as the user's message in the chat and terminal UIs. This is confusing — users see a wall of prompt text where they expect to see the compact slash command they typed.
