GitHub Copilot authentication is not indicated in /connect

Description

After authenticating GitHub Copilot with opencode auth login, opencode auth list shows the stored credential but the TUI /connect dialog does not show GitHub Copilot as connected. The provider endpoint derives its connected IDs only from instance-cached runtime provider state, so credentials added after that state initializes are omitted.

Steps to reproduce

1. Initialize an OpenCode provider instance.
2. Run opencode auth login and authenticate GitHub Copilot.
3. Confirm GitHub Copilot appears in opencode auth list.
4. Open the TUI and run /connect.
5. Observe that GitHub Copilot has no connected checkmark.
