MERGE Gateway uses the fallback provider icon

Description

MERGE Gateway is available through models.dev and has an official SVG there, but its ID is missing from OpenCode's provider icon sprite. Provider and model lists therefore display the generic fallback icon instead of the MERGE Gateway mark.

1. Connect the MERGE Gateway provider.
2. Open a provider or model list that displays provider icons.
3. Observe that MERGE Gateway uses the generic fallback icon.

Same issue applies more broadly: ANY custom provider added via @ai-sdk/openai-compatible (e.g. Command Code GOAT, self-hosted gateways, proxies) always falls back to the generic "synthetic" icon, and there's no way to set one — ProviderConfig has no icon field and ProviderIcon (packages/ui/src/components/provider-icon.tsx) resolves hardcoded sprite ids only.

A configurable provider icon field (matching sprite id, emoji, or URL/data-URI) would fix this whole class of issues, not just MERGE Gateway.
