fix(opencode): recover stale encrypted reasoning state

What does this PR do?

Existing Responses sessions can become unusable when stored OpenAI reasoning continuation state is rejected with invalid_encrypted_content.

This detects that failure before any output is emitted, removes stale itemId and reasoningEncryptedContent from both the retry history and persisted reasoning metadata, then retries once using the visible conversation history. Other provider metadata and normal encrypted reasoning replay are preserved.

How did you verify your code works?

• bun typecheck
• bun test test/session/processor-effect.test.ts — 18 passed
• Repeated the full processor effect suite 5 times successfully
• oxlint — 0 errors

Regression tests cover direct APICallError recovery, persisted metadata cleanup, preservation of visible reasoning and unrelated metadata, and the one-recovery-attempt limit.
