[D6] Streaming exemption precision + bind_guard fallback

Two fixes:

1. Match exact method+path patterns for streaming timeout exemption (http.go ~640) so non-streaming POST …/wait still times out.
2. Log/derive bind fallback from listener instead of hardcoded 127.0.0.1:8080 (bind_guard.go ~61) so custom-port daemon MCP calls succeed.

Acceptance criteria

• Non-streaming POST …/wait still times out; custom-port daemon MCP calls succeed.

Impact analysis

Middleware + startup diagnostics only.
