chore: install code orchestrator skill pack

Summary

• replaces the standalone phase-orchestrator with the complete five-skill workflow
• adds write-phase-plan, phase-implementer, phase-reviewer, and phase-final-reviewer
• preserves the upstream MIT license and provenance at kousun12/code-orchestrator commit 989e49ba7b0d8cb922010272abe6748dd68a4adb
• uses the Linux custody test helper’s existing bounded 8-second child-close window; the prior 5-second observation window exactly matched the native 5-second deadline and failed twice under full-suite load even though the focused test passed

The production custody deadline and expected zero exit are unchanged.

Validation

• repository-native validation passed for the skill task tree
• bun test scripts/authority-supervisor-runtime.test.ts (11 passed)
• YAML, attribution, provenance, required files, and task-only paths passed the fleet audit
• required PR CI revalidates the current branch on Linux and macOS
