docs: require first-reader clarity

Summary

• add the shared first-reader clarity policy for public prose
• require self-contained context, a direct thesis, accurate source framing, and explicit limits
• keep private implementation details, internal reasoning, and editorial process out of reader-facing writing

Validation

• git diff --check; the full suite reached 1,980 passing tests, and both load-sensitive timeouts passed on a focused 31-test rerun
