Allow larger free Smart requests within provider budget

Free Smart requests could be rejected before inference because a worst-case JSON size estimate was compared with the Fast provider allowance.

• Give free Smart requests a $0.10 provider-cost ceiling while Fast stays at $0.01. Keep the $0.50 per-IP and $100 shared daily ceilings.
• Admit requests using the provider budget and allow a smaller reservation when daily allowance is nearly spent.
• Return completed fast answers when later Smart reviews cannot fit; report missed reviews in usage.escalation_failed.
