Adds docs/superpowers/notes/2026-09-13-grid-not-connected-investigation.md recording the finding: gridState()'s directionKnown staying true during NOT_CONNECTED is a deliberate day-one design choice (PR #9), and a full-history scan of production data (34.36M rows, ~2 years) confirms grid_mode=NOT_CONNECTED has never actually occurred on this install, so it's provably dead code here.
