Not positive, but I think the Github API does not support 2-factor authentication.

Update: I was incorrect. The API does support it. https://developer.github.com/v3/auth/#working-with-two-factor-authentication

We were able to get two-factor auth working by catching GitHub's error and using the Github::Client::Authorizations directly to supply the special headers like so:

rescue ::Github::Error::Unauthorized => e
  if e.inspect.to_s.include?('two-factor')
    Github::Client::Authorizations.new(
      basic_auth: "#{user}:#{password}",
      endpoint:   api_endpoint,
      site:       site_url,
      ssl:        {:verify => false},
      headers:    { "X-GitHub-OTP" => two_factor_code }
    )
  end
end
