Plan v3 §5.3: slices without an explicit mode ran with bypassPermissions plus --dangerously-skip-permissions. Default is now acceptEdits; bypass requires ORGX_AUTOPILOT_BYPASS_PERMISSIONS=1. Codex --full-auto is the sandboxed workspace-write mode and is unchanged. Checks: npm run build:core; full hook suite 746 pass, 0 fail. Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>

The 0.7.40 publish job ran on floating Node 24 (now 24.21), where better-sqlite3 11.x aborts at process exit after all tests pass. CI tests Node 18/20/22, so publish on 22. 0.7.41 carries the 0.7.40 changes (#317). Co-authored-by: Claude Opus 5.5 <noreply@anthropic.com>
