docs: V2 close-the-loop hygiene after #82–#91

Summary

Part 1 hygiene after V2 scaffold PRs #82–#91 landed on main.

Docs no longer say “Phase 0 next / start at #68.” They describe scaffold on main and point next work at hot-path deepening + signature demo.
