Plan v3 Stage 0 (§5.5, §5.6, §5.10, A03, §2): - Durable outbox deletes only via settleOnAck (operation id + sha256 of the exact bytes posted); 409 run_already_terminal settles as superseded. task.failed is now accepted server-side (hopeatina/orgx companion PR). - Hook spool replay posts one Work Graph report per real source client instead of labelling every client's records as codex. - Package files now include emit-execution-graph.mjs: published 0.1.20 cannot import orgx-work-graph-reconcile.mjs (ERR_MODULE_NOT_FOUND), so npm-installed peers never uploaded the spool. verify-plugin now fails when a packed module imports an unpacked sibling. - Release workflow stamps manifest_fingerprint from the packed file set (and signature when ORGX_MANIFEST_SIGNING_KEY is set). - Correct the stale claim of a Claude plugin Stop-hook path. Ships #60 (settle receipts on digest-bound ACK; fix the published package) and #61 (advance the shared-spool cursor only on a digest-bound ACK). Versions aligned across package.json, package-lock.json, .codex-plugin/plugin.json and plugin.manifest.json; the peer test's release fixture follows the version.
