Ships #60 (settle receipts on digest-bound ACK; fix the published package) and #61 (advance the shared-spool cursor only on a digest-bound ACK). Versions aligned across package.json, package-lock.json, .codex-plugin/plugin.json and plugin.manifest.json; the peer test's release fixture follows the version. Ships #60 and #61 since v0.1.20: receipts settle only on a digest-bound ACK, the shared-spool cursor advances only on a digest-bound ACK, and the published package is fixed.
